Enrollment Process
Thank you for enrolling your child in the 1st Choice
Before and After School Kare Program (A.S.K.). Our goal is
to make sure your child has a fun yet educational experience
while staying at the A.S.K. program.
The enrollment process made simple
- Print then fill out the enrollment forms
below and return to the A.S.K. director at
your child’s school. Please do not turn them
into the front office of your child’s
school; they do not handle the A.S.K.
program.
- We will need a copy of your child’s
immunization records. If you do not have a
copy, ask the front desk at your school to
make a copy for you. Below is a copy of a
blank immunization form. Please make sure
your form matches this form. If not, please
have your child’s information transferred to
the form provided below by you or your
doctor.
- We will NOT need a doctor’s signature on
any of the forms unless your child needs to
take “Over the Counter” medication.
- On the “Authorization for Draft Form”,
simply put “Card Number on file” in the
space that asks for your credit card number;
please sign and date.
